Complex Care Nurses
The Healthy Hyde Complex Case Nurse Team provides a range of proactive services to support frailty across the 8 GP practices within the PCN. The main aim of the service is to give everyone living with frailty more choices and control over their care.
There are 12 care homes within the Hyde PCN area and each care home has a dedicated nurse attached to provide a proactive weekly care home ward round. The nurses complete a comprehensive geriatric assessment, offer advanced care planning discussions and work alongside a wide range of health and social care professionals to provide person centred optimised care.
We provide home visits for housebound patients living with complex health needs. We provide comprehensive holistic assessment and assist in case managing patients to maximise their quality of life within there own homes and community. We have a collaborative care model whereby we engage with a range of health and social care professionals to enable this to happen.
