Dementia Service

The dementia case management service was established in January 2023 and initially trialled with one GP practice. Following a successful trial and feedback, the service was rolled out to all other 7 GP surgeries that are part of Hyde Primary Care Network. This service aims to provide proactive, person- focused assessment & management of those living in their own homes with a dementia diagnosis. A named nurse formulates a personalised care plan to ensure that the right care is accessed in the right place at the right time. We work with the wider multidisciplinary team to support people’s health and social care needs to provide consistent focused care and management.
